Job Description Job Description Wraparound Assessor Summary The
Wraparound Assessor is responsible for conducting timely,
high-quality initial assessments and reassessments for
children/youth enrolled in Wraparound services. This role evaluates
the needs, strengths, risks, and service eligibility, and
translates findings into clear recommendations for plans of care.
The Wraparound Assessor also collaborates closely with Wraparound
Facilitators and Wraparound Licensed Clinical Director to ensure
assessments inform service linkage and effective clinical services.
This position reports to the Wraparound Licensed Clinical Director.
Position Type Full-time, Non-Exempt Essential Functions Complete
individualized assessments including a Child and Adolescent Needs
and Strengths Survey (CANS), Independent Behavioral Health
Assessment (IBHA) form, and other assessments as needed in the
family’s home or community. Use assessment tools to inform the POC
and crisis/safety planning in coordination with the Wraparound
Clinical Director. Collaborate with wraparound facilitators to
translate needs/strengths into measurable goals, interventions, and
progress indicators for the POC; ensure assessment-driven
recommendations are reflected in POC updates and service
authorizations. Supports CSoC principles and values. Promotes
services that are community-based, evidence-based, and/or
evidence-informed. Assures all assessments and reassessments
assigned are completed prior to the end of the authorization
period. Complete The Louisiana CSoC Wraparound training and
certification process as developed by the State and OBH. Ensures
all assessments are completed by deadlines determined by VOASCLA
and/or Magellan. Maintain confidentiality as it pertains to all
aspects of clients’ care, staff, and Volunteers of America.

Supervisory Responsibility N/A Required Education and Experience
Master’s degree from accredited university or college in human
services, social or behavioral science required. Licensed as a
mental health professional required. Three (3) years of experience
in the human services field required, preferably with different
systems, including school, behavioral health, child welfare,
juvenile justice, health, and delinquency population. Experience
with wraparound services, Medicaid-funded programs, or behavioral
health systems preferred. Other Requirements Must maintain a valid
Louisiana driver’s license. Must maintain a valid Counseling or
Social Work license in the State of Louisiana. Complete and
maintain CANS certification. Physical Demands While performing the
duties of this job, the employee is frequently required to stand;
walk; sit, use hands, handle, or feel objects, tools or controls;
reach with hands and arms; climb stairs; balance; stoop, kneel,
crouch or crawl; talk or hear; taste or smell. The employee must
occasionally lift and/or move up to 50 pounds. Specific vision
abilities required by the job include close vision, distance
vision, color vision, peripheral vision, depth perception, and the
ability to adjust focus. While performing the duties of this job,
the employee can sometimes be exposed to weather conditions. The
noise level in the work environment is usually moderate. Other
