Job Description:

AccentCare is seeking a dedicated Social Worker to join our team of healthcare professionals to assist in providing comprehensive, compassionate care to our clients. In this role, you will work closely with patients and their families to offer psychosocial support and coordinate with medical professionals to ensure personalized care plans are seamlessly integrated into the patients overall health management. This position requires a deep understanding of the emotional and practical challenges patients face and the ability to provide expert support and guidance through their care journey. Assess patient needs, situations, strengths, and support networks to determine their goals Develop comprehensive care plans in conjunction with healthcare professionals Provide information on resources to assist clients with healthcare challenges Support clients in coping with stress and emotional concerns related to their conditions Advocate for clients with healthcare providers, social agencies, etc., ensuring access to services they need Follow-up with clients to monitor progress and make necessary adjustments to care plans Strong interpersonal and communication skills Ability to manage multiple cases efficiently and effectively Deep understanding of healthcare systems and resource networks Expertise in crisis intervention techniques Capability to work both autonomously and collaboratively in a fast-paced environment Medical Insurance Dental Insurance Vision Insurance 401k Licensed Clinical Social Worker (LCSW) or eligibility for certification Valid driver's license and reliable transportation, as some travel may be required
