Job Description:
Description Summary: Responsible for the overall comprehensive
care provided to the patients in the CHRISTUS SETX Health System.
NP will coordinate care with the Medical Director utilizing
training with the scope of their practice, conduct patient
education and ensure accurate and timely documentation in
accordance with the regulating agencies and organizational policy.
Will also participate in community agencies and organizational
policy; participate in community events and marketing opportunities
involving the CHRISTUS SETX Health System. The NP will coordinate
patient management and individualize a plan of care for each
patient, working closely with the Medical Director and other team
members. Associate will take medical histories, perform physical
exams, order and interpret laboratory studies and provide treatment
to patients with chronic diseases. Working closely with the RN and
Medical Director to ensure follow up calls are made, assist in
identifying additional visits or interventions that may be needed
as well as managing/adjusting medications and educating patients.
Responsibilities: Assesses critical elements of the patients
history, interprets test results, and institutes practice protocols
with a high degree of independence Partners with physician
colleagues to diagnose and manage patients using guidelines and
approved standards of care. Leads the health care team with expert
advice to case managers, nurses, physicians, social workers, and
other health care members as appropriate Communicates findings and
plan of care to physician and other members of the healthcare team
through progress notes and verbal communication. Interprets and
follows up on test results and response to treatments and
medications. Evaluates patients response to the plan of care, and
revises the plan as needed, collaborating with a physician as
required. Identifies patient/family educational needs and in
collaboration with appropriate staff, provides patient/family
education Acts as an advocate for the patient/family, encouraging
participation in decision-making and care. Tracks clinical
performance outcome data Performs advanced procedures for which
competency has been verified Assess learning capabilities and
readiness of population or individuals, and tailors education to
meet age, developmental, and educational needs. Prioritizes
learning needs and documents them accordingly Ensures that time
frame and subject matter is appropriate for target
audience/individual Utilizes appropriate teaching materials
(handouts, audiovisuals, demonstration, etc.) and documents use and
patient outcome. Initiates, designs, and completes educational
programs for patients, families, and target audiences. Communicates
advanced knowledge and skills related to patients, families, and
staff at the organizational, community, and national level. Adheres
to agreed upon format/protocol for documentation of records.
Maintains appropriate level of documentation to promote/support
level of complexity when compared against/to service fee charges.
Documents telephone calls with patients and specialists - Documents
in concise, clear, logical, and legible manner. Identifies the
knowledge and skills needed through self-assessment, peer review,
and supervisory review Participates in a variety of educational
activities in order to maintain and increase competency. Maintains
licensure/certification Uses continuing education opportunities as
a basis for expanding knowledge and improving clinical skills, and
incorporate these into the daily practice. Develops plan for future
professional activities. Member of appropriate professional
organizations. Provides constructive feedback as it relates to
performance, patient flow, and patient care issues. Provides
educational activities for nursing staff as needed Identifies
barriers to patient care flow or delivery, and assists in
formulating corrective action Serves as a role model for patient
interaction and conflict resolution for hospital staff. Works
collaboratively with health team members to ensure continuity of
care and complete delivery of prescribed care. Serves as a liaison
to nursing and other health care specialists in the hospital
Assesses and manages all medication therapies according to
established protocols, practice guidelines, standards, or
algorithms as warranted by patients diagnosis or changing medical
condition. Provides proper notification for all absences or
tardiness, scheduled shift, and scheduled time off in accordance
with hospital policy. Consistently uses products, supplies, and
equipment in an efficient manner, keeping waste within practice
limits as observed by peers and administration Regularly performs
job tasks in accordance with hospital policy and procedures,
including appropriate use of equipment, appropriate use in wearing
of physical barriers and safety equipment. Demonstrates a complete
knowledge of body mechanics by consistent use in the work setting
as evidence by no injuries sustained as a result of improper body
mechanics in the evaluation period. Responds to codes in accordance
with emergency procedures. Regularly maintains work area and
equipment in a neat and orderly manner, assists in the cleaning of
other work areas, and corrects any malfunctioning equipment or
environmental conditions as observed by administration.
Requirements: Education/Skills Graduate of a State Board and/or NLN
accredited School of Nursing. Graduate of a State Board and NLN
Accredited Advance Nurse Practitioner program. State certification
required; national certified preferred. Masters Degree required.
Effective verbal and written communication skills to communicate
with diverse populations including physicians, employees, patients
and families. Leadership skills to direct others toward the
successful treatment of patients. Analytical skills to evaluate
patient status and healthcare procedures/techniques and to monitor
quality of care. Fiscal skills to monitor and control costs and
revenue. Ability to conceptualize, develop and implement new ideas
or systems. Able to exercise sound judgment and discretion while
performing duties. Experience One year full time post licensure
clinical nursing experience preferred. Advanced Nurse Practitioner
experience preferred Licenses, Registrations, or Certifications
Current license to practice professional nursing in the State of
Texas required. Current certification to practice as an Advance
Nurse Practitioner in the State of Texas required. Certification as
Nurse Practitioner by the American Nurses Credentialing Center
(ANCC) or the American Academy of Nurse Practitioners (AANP). AHA
BLS within two (2) weeks of employment required. AHA ACLS required.
Computer Competence - within four (4) weeks of employment Work
